The practitioners associated with SFM are dedicated to providing the highest degree of family health care. Established in 2003, we operate under the philosophy that all patients should be treated individually. At SFM, you are not a number and will be treated professionally at every visit. 

We take the time to properly diagnose our patients and will refer to a specialist when needed. We are not afraid to suggest homeopathic remedies, and when the situation truly calls for prescription drugs we prescribe with precision.

To serve you better, please have a copy of your current insurance card and photo id with you to your appointment. Not having these items could cause a delay in your appointment and may even be asked to reschedule.

 Patients are responsible, at the time of service, for any copay, deductible and/or remaining balances on their account. We allow 60 days for insurances to pay their portion of the claim. If after that time the insurance has not paid, the balance will be turned over to the guarantor for the account. Payment for all claims is expected within 90 days. In addition to cash and checks, we accept Mastercard, VISA, Discover and American Express. All minors must be accompanied by a parent or a legal guardian.
